Creekside Performance Center: Cedar Grove is located at 11 Phoenix Avenue, Cedar Grove, Wisconsin 53013,  is accepting patients. We are providing our services in a private suite within the Bella Vida Salon building! (click address above for Google map directions!). This space allows us to bring high-quality chiropractic and wellness care closer to the community of Cedar Grove. 

As of 11/24/25, we are accepting commercial health insurances. We are in network with almost all commercial insurances.

Insurance and payments: 

At Creekside Performance Center: Cedar Grove, we are committed to making your care experience as smooth and transparent as possible. For patients using commercial insurance, all claims will be submitted directly to your insurance carrier on your behalf. Once your claim is processed, any remaining balances—such as deductibles, co-pays, or co-insurance—will be billed to you.

While insurance coverage can vary based on your individual plan, we will do our best to provide a good faith estimate of expected costs prior to or at the time of service. This estimate is not a guarantee of coverage but is intended to help you understand potential out-of-pocket expenses.

Can my kids come in by themselves for treatment?

During the initial visit, it is required to have a parent or guardian present, but if a consent form is signed, a child may come in and receive treatment without a parent or guardian present.
